Application Process

There are two forms that need to be completed and turned in by the dates indicated below;

1. Internship Process for Supervisors

This three page document is to be given to your prospective site supervisor. This document must be read, understood, and signed by your site supervisor prior to your registration into this course.

2. Application Forms for students following the NEW program and OLD program

The majority of this packet can be completed by you. There are a number of items that you must complete and return with the packet (indicated on the first page of this packet). Please note that this form should be signed by your site supervisor. If you cannot meet in person, this form may be faxed back and forth between you and the site supervisor to attain signatures. Be sure to get the correct signature, the "site supervisor" is the person that will be observing you on an ongoing basis, commenting on your online journals and completing your final evaluation.

3. Turn in both forms by hand to Amy Gyorkos by the following dates;

• Spring Semester: Internship Application: 1st Friday in November
• Summer Semester: Internship Application: 1st Friday in March
• Fall Semester: Internship Application: 1st Friday in June
