Letters of Recommendation
                                                               

If you want me to write a letter of recommendation for you, it is important that you know that in the letter I will share your letter grade and class ranking for the course(s) you took with me.  If you feel that your performance in my courses do not accurately reflect your abilities, you may want to reassess whether I am the best person to serve you in writing a letter of recommendation. If I am to write you a letter, I also need the information listed below.  Some of the information (resume and personal statement) help me when commenting on nonacademic issues.

  1. submission deadlines, contact addresses and relevant materials (recommendation forms) for each school to which you are applying.
  2. undergraduate transcripts (unofficial will be fine)
  3. GRE scores
  4. a recent resume or summary of relevant extra-curricular activities (related work, volunteer work, athletics and organizations)
  5. a copy of your personal statement if completed.  Otherwise, some indication about why you are choosing communication disorders as a profession would help.
  6. You must complete a release so I have permission to send the letter.  Here is a copy of the waiver (release form:pdf)
